Neutrophils can promote graft inflammation through a form of programmed cell death termed "NETosis". How alloantibodies contribute to neutrophil responses in grafts is not clear neither the expression of NETosis in peripheral blood of KTR. Aim and Methods: The purpose of this exploratory study was to evaluate NETosis in peripheral blood from adult kidney transplant recipients (KTR) with biopsy confirmed acute antibody mediated rejection (AAMR; n=10) compared with KTR without any immunologic event (IE) (controls, n=13). Spontaneous and LPS-induced NETosis was evaluated by fluorescence spectrometry with the Sytox Green staining and with confocal microscopy. The amount of NETs was quantified as the number of the fibrillar structures in which chromatin and MPO colocalized, divided by the number of cells in 6 high power fields per experimental condition. The expression of myeloperoxidase and Histone 3C (H3C) was measured by the MFI normalized for the area of every NET. Results. The main findings are depicted in Table 1.

Total AAMR (n=10) Controls (n=13) P value
Nets/cells ratio LPS (SD) 0.320 (0.23) 0.287 (0.19) 0.345 (0.27) 0.569
Nets/cells ratio spontaneous (SD) 0.368 (0.21) 0.374 (0.20) 0.364 (0.23) 0.912
MPO MFI LPS (SD) 1484 (364) 1461 (393) 1502 (354) 0.795
MPO MFI Spontaneous (SD) 1398 (348) 1301 (285) 1473 (383) 0.250
H3C MFI LPS (SD) 214 (129) 288 (156) 156 (64) 0.028
H3C MFI Spontaneous (SD) 195 (110) 216 (110) 179 (112) 0.451
NETs by fluorescence spectrometry LPS (SD) 1779 (611) 1739 (601) 1810 (641) 0.788
NETs by fluorescence spectrometry Spontaneous (SD) 1821 (673) 1952 (667) 1721 (687) 0.427

Figure 1: Representative image of NETs by confocal microscopy. Conclusion: NETosis was observed similarly in both groups. NETosis may translate persistent activation of neutrophils despite inmunosuppression.
