Perioperative Results of Lung Transplantation in a Newly Established, High-Volume Lung Transplantation Center in China

*Purpose: The number of patients with end-stage lung disease is huge in China, lung transplantation is the only effective method. By presenting the perioperative results of lung transplantation in a newly established, high-volume lung transplantation center in China, we aim to introduce the Chinese experience, and improve future work.

*Methods: The perioperative data of recipients who underwent lung transplantation in our lung transplantation center from March 2017 to June 2019 were retrospectively analyzed.

*Results: A total of 203 cases of lung transplants were performed, including 202 adult lung transplants and 1 in children. In adult, there were 178 male and 24 female recipients. The average age of recipients was 58.2 years (21-76 years), the mean BMI was 21.8 (12.3-30.8). Among the primary disease types, interstitial lung disease accounted for the highest proportion (143 cases, 70.8%), followed by COPD (28 cases, 13.9%), bronchiectasis (12 cases, 5.9%) and others (19 cases, 9.4%). A total of 134 single lung transplantation (1 case received coronary artery bypass grafting simultaneously) were performed, and 68 bilateral cases were performed. The average time of single lung transplantation was 229 minutes (110-575 minutes), the average blood loss was 517 ml (30-6000 ml), and the average time of bilateral lung transplantations was 350 minutes (193-590 minutes), the average blood loss was 902 ml (50-5000 ml). During the perioperative period, 148 cases were assisted by ECMO, accounting for 73.3%. The overall perioperative survival rate was 80.7% (163/202). Of the 39 patients who died, 31 (79.5%) died of severe infection, 5 (12.8%) died of cardiogenic disease, and 3 (7.7%) died of severe bleeding. Compared with the early completion group by setting June 30, 2018 as the cut-off operation time point, the time of single lung transplantation was significantly reduced (215 vs 243 min, p=0.029), no significant change in blood loss (494 vs 540 ml, p=0.764). No significant change in neither time (344 vs 357 min, p=0.465) nor blood loss (785 vs 1042 ml, p=0.286) for bilateral lung transplantations. There was no significant difference of the overall perioperative survival rate (79.0% Vs 82.4%, p=0.546) between two groups.

*Conclusions: Lung transplantation program in China has its own characteristics. Considering the higher percentage of elder recipients, more common in recipients with critical condition, and the difficulty of maintenance and transportation of the donor lungs from long-distance, it is necessary to establish a reasonable workflow and quality control system. The initial experience of our lung transplantation center might provide reference value to other lung transplantation teams in both China and around the world.
