*Purpose: In the European multicentre “ChilSFree” study we aimed to characterize longitudinal dynamics of soluble immune mediators during the first year after pLTx.
*Methods: Using Luminex-based multiplex technique, we measured 50 cytokines/chemokines, growth and adhesion factors in recipient plasma at visits: before (V0), 1 (V1), 2 (V2), 3 (V3), 4 (V4) weeks, 3 (V5), 6 (V6) months and 1 year after pLTx (n>205).
*Results: Multigroup comparisons revealed main changes in plasma secretome to appear within V0-3, while later visits were clustered by their secretome signatures independently on the visit kinetics, likely reflecting different recovery patterns. For instance, one of these secretome signatures was characterized by the absence of pro-inflammatory CXCL8/9/10/12, CCL7, IL-17 mediators and reduced liver enzyme (ALT, AST, GGT) levels, and, hence, might be predictive for a better outcome after pLTx. We also identified IS regimes which were linked to diminished inflammation and rejection rates. Some of them showed sex-dependent patterns.
*Conclusions: Plasma secretome signatures might act as a biomarker for potential outcome after pLTx. This information might be useful for improved therapeutic options.
