Background: Donor extraction times for kidney allografts are associated with inferior kidney outcomes. We hypothesized that that longer donor liver extraction times would be associated with recipient early allograft dysfunction (EAD) and/or acute kidney injury/failure (AKI/F). Methods: Extraction times from 632 deceased donor liver transplants from DBD donors (n=632) were reviewed and examined for associations with EAD and kidney injury as measured by RIFLE criteria. Results: 174 Patients qualified as having EAD (27.7%) and 125 had AKI/F (19.9%). Extraction time was not predictive for EAD (p=0.08) or AKI/F (p=0.9). Median extraction was 31 min (IQR =25-38). None of our patients had all 5 variables of injury (AST, ALT, Tbili, INR, and Screat >2x baseline). The average extraction time for patients with any combination of variables of injury fell between 30.4 and 32.7 minutes. The average extraction time for patients with no variables of injury was 32.1 minutes. Conclusions: There was little variation in extraction times, which speaks to the homogenous liver extraction techniques of a single center, in contrast to the often varied techniques and recovery personnel for kidney extraction. Unlike published findings correlating longer extraction time of kidneys with worse kidney function/outcomes, liver extraction times were not associated with either EAD or AKI.
