Transplantation of Restored Kidneys from Living Related Donors after Small Renal Tumor Resection: Report of 5 Cases

  • No Tumor Recurrence After Renal Cell Carcinoma Resected Kidney Transplantation – 5 Year Follow Up

Purpose: We launched a clinical trial registered at ClinicalTrials.gov that aimed to study the utility and safety of restored kidney transplantation between family members using therapeutic donated kidneys.

Subjects and Methods: Incidental small renal tumors were identified during the examination of two men and three women undergoing preoperative donor workups for living related kidney transplantation. Living related kidney transplantation was performed using the donor kidneys, each of which was partially nephrectomized and restored, in three ABO-compatible cases and two ABO-incompatible cases.

Results: The small renal tumors were identified as renal cell carcinomas in three cases and angiomyolipomas in two cases. The first recipient with a functioning graft died suddenly of cardiac arrest 2.5 months after surgery. The four remaining recipients (two of whom underwent simultaneous bilateral nephrectomy for polycystic kidneys and one of whom underwent nephrolithotomy) have experienced a satisfactory quality of life. Their most recent serum creatinine levels ranged from 0.89 to 1.39 mg/dl, without tumor recurrence at 0.5 to 5.1 years after surgery.

Conclusions: Successful ABO-incompatible restored kidney transplantation using therapeutic kidneys, despite a limited experience of only two cases, may provide novel options for living related kidney transplantation; however, the very challenging procedure requires experienced surgeons at well-equipped centers.
