*Purpose: For increasing pancreas donor pool, the use of pediatric donors for pancreas transplantation is considered as a suitable option. However, the following questions still remain because of the risk of surgical complications, in particular of thrombosis and pancreas graft volume. In this study, we analyzed our result of pancreas transplantation from pediatric donor.
*Methods: All pancreas transplantations from donors younger than 18 years, between June 1992 and October 2019, were included. These recipients were divided into two groups according to the age of donor (≤15 years old, >16 years old) and we analyzed the influence of donor age to graft outcomes.
*Results: 98(98/453,21.6%) cases were performed from pediatric donor in all pancreas transplantation. Among them, 48 cases(49.0%) were under 15 years old and 50 cases(51.0%) were over 16 years old. The mean age and weight of recipients were 37.46 ±11.19 years and 56.83 ±10.52kg. 34 cases(34.7%) were SPK, 38 cases(38.8%) were PTA. PAK and SPLK were 13 cases(13.3%), respectively. The mean age of donors were 14.47 ±3.71(3-18) years and 56.16 ±17.76(15-90)kg. The weight of graft pancreas was 151.36±45.99(52-252)g. Overall graft survival was not significant different compared to those from adult donors ( 90.3% vs. 91.3% at 1 year, 81.1% vs. 81.6 at 5 year, and , 76.5% vs. 73.6% at 10 year, p=0.902). When we compared weight of donor and pancreas graft were significantly different according to their age subgroup(48.22±20.74 vs. 62.55±11.78, p=0.002, 135.94±48.15 vs. 169.09±36.50, p=0.001). However, the graft survival of subgroup was not significantly different (p=0.620).
*Conclusions: The outcomes of pancreas transplantation from pediatric donors may be comparable to those of pancreas transplantation from adult donors. Therefore, pancreas transplantation from pediatric donors can be suitable options for expending donor pools for patients waiting pancreas transplantation.
