*Purpose: Our study aimed to shed light on the role of serum levels of MMP and their inhibitors- TIMP in renal allograft rejection.

*Methods: This was a retrospective study done in patients who underwent renal transplant in a tertiary care center in the year 2012. The patients were divided into two groups – Rejecters and Non-Rejecters based upon histopathological diagnosis of acute transplant rejection (within one year after transplantation). Of the total 250 renal transplants, 38 were rejecters. 32 non-rejecters and a control group comprising of 20 healthy transplant kidney donors were also selected randomly. The values of MMP1, MMP2, MMP9, TIMP1 and TIMP2 were analysed using sandwich ELISA (enzyme linked immunosorbent assay) in serum samples stored at -80⁰C (collected within one month of rejection episode in rejecters and at 12th month after transplant in non-rejecters).

*Results: The study included 38 rejecters (32 men and 6 women, mean age-32.8±9.5 years), 32 non-rejecters (28 men and 4 women, mean age-36.1±11.8 years) and 20 controls (4 men and 16 women, mean age-42.5±12.7 years). Compared to the control population, renal transplant recipients showed significantly higher median serum values (p<0.001) of MMP1 (5.3 & 10.2 ng/ml), MMP2 (0.9 & 3.5 ng/ml), MMP9 (68.06 & 379.89 ng/ml), TIMP1 (0.0 & 11.2 ng/ml) and TIMP2 (0.0 & 13.2 ng/ml). Rejecters and non-rejecters when separately compared with the control group also showed significantly higher values of these proteins. However, there were no significant differences (p>0.05) in the median serum values between rejecters and non-rejecters – MMP1 (10.5 & 9.2 ng/ml), MMP2 (3.7 & 3.2 ng/ml), MMP9 (379.89 & 437.29 ng/ml), TIMP1 (20.3 & 3.0 ng/ml) and TIMP2 (13.2 & 9.7ng/ml).

*Conclusions: Both MMPs and TIMPs were significantly higher in renal transplant recipients compared to the healthy controls. We could identify dysregulation of the MMP/TIMP system in both transplant rejection and stable graft. However, the levels of these factors were not significantly different between acute rejecters and stable grafts in first year post transplantation. Hence long term studies are required to identify the imbalance of MMP/TIMP system in chronic graft rejection.
