Prediction of Delayed Graft Function By Means of a Web-Based Calculator in Living Related Kidney Transplant: Mexican Single Center Experience

Background: Delayed graft function (DGF) is associated with, poor graft survival. Irish validated a model to predict DGF for deceased donor transplants. The model was used to program a web-based DGF risk calculator (www.transplantcalculator.com) but it has not been validated in living related donor kidney transplantation. We analyzed the performance of this calculator in living related donor kidney transplants.

Methods: 108 living related donor kidney transplant (2003-2013) were included for analysis. DGF risk was assessed using the web-based calculator. Transplant outcomes included were: DGF and graft survival. All patients received induction (96.3% basiliximab, 3.7% thymoglobulin), CNI, MMF and steroids. DGF discrimination was measured by ROC curve c statistic. Kaplan-Meier/Log rank test was used for graft survival. P < 0.05 was statistically significant.

Results: Donor, recipient and transplant characteristics are depicted in Table 1. Overall DGF rate was 9.3% whereas average individual web-based calculator DGF was 6.9%. These differences were not statistically significant. Mean calculated DGF risk was similar in patients with DGF and without DGF (6.9%) (p=0.995). The area under the ROC curve to predict DGF was 0.515 (p=0.995). Overall 5-year/10-year graft survival was 92% and 78%. Grafts with DGF had worse 5-year/10-year graft survival (80% and 53%) than those without DGF (95% and 87%) (p=0.03)

Conclusions: DGF calculator have a poor degree of discrimination and cant predict DGF risk development in living related donor kidney transplant. DGF has a deleterious effect in graft survival.
