Pancreas Retransplantation: Outcomes of This Technical and Immunological Challenge over 38 Patients

Introduction

Pancreas transplantation is now a mastered technique for patients suffering from severe diabetes. However, a significant number of grafts are lost for technical and immunological reasons.

Pancreas retransplantation is can be option in selected patients, but remains a high risk intervention. The goal of this study was to review our experience in pancreas retransplantation, to explore its surgical and immunologic outcomes.

Methods

All pancreas retransplantation that occured between 1996 and 2017 in Nantes were included. Graft survival and causes of graft loss were evaluated, as well as patients survival.

Results

From january 1987 to feburary 2017, 38 pancreas retransplantations were performed by the urologic surgeons in the CHU of Nantes. There was 9 female and 29 male, with a mean age of 42 years.

Graft survival was 64.5% at 5 years, 44.8% at 10 years. 11 patients (28,9%) lost their second pancreas, 3 from veinous thrombosis, 2 from chronic rejection.

Patients survival was 85.2% at 5 years, 74% at 10 years. Only one death was pancreas related (gas gangrene).

As for the kidney transplant, 5 patients had to go back on dialysis (4 for chronic kidney rejection, one after immunosuppressors arrest).

Discussion

A second pancreas transplantation is an acceptable option for diabetic patients that lost their first graft, with a good graft survival and a few impact on the kidney transplant.
