*Purpose: Kidney transplantation remains the treatment of choice for end stage renal disease as it improves mortality and morbidity. Outcomes of incarcerated ESRD patients who have received kidney transplants have rarely been reported. We studied kidney transplant outcomes in incarcerated ESRD transplant patients who received deceased donor kidney transplantation at our center.

*Methods: We retrospectively reviewed kidney transplant outcomes in incarcerated ESRD patients who received deceased donor kidney transplants at our center. Patients received thymoglobulin induction and maintenance with tacrolimus, mycophenolate and prednisone. Kidney transplant outcome was defined by acute rejection rate, failed allograft and patient death.

*Results: 9 incarcerated ESRD patients received deceased donor kidney transplants with follow up ranging from 2-15 months. Incarcerated ESRD patients had longer wait times with median wait time of 10.5 years. No patient developed DSA or experienced acute rejection, graft failure or death

*Conclusions: Kidney transplantation in incarcerated ESRD patients has limited clinical data. Although post -transplant follow-up was limited, these findings suggest that kidney transplantation is an option for incarcerated ESRD patients. Studies with longer term follow-up are warranted
