BACKGROUND: The rates of morbidity and mortality in patients with a failed kidney transplant are higher than dialysis patients who had never been transplanted. The aim of this study was to establish if there is any difference in inflammatory status between patients on dialysis with non-functional graft versus non-transplanted patients.

METHODS: Case-control study. Cases were patients with retained non-functional graft, without surgical procedures or infections during previous 2 months. Control cases were paired by age, gender, ESRD etiology, and time on dialysis. Cytokines were measured by luminometry (Luminex). Ultrasensitive C-reactive protein was determinate by ELISA. Chi Square test, Student T test, and Mann-Whitney U test were used.

RESULTS: A total of 44 cases and 70 controls were included. The characteristics are showed in Table.

VARIABLE Failed Graft (n=44) Controls (n=70) p
Age 34.5±9.8 35.3±11.8 0.703
Time on dialysis (years) 2.82 (0.4-19.5) 3.13 (0.53-12.8) 0.817
BMI 22.7 ± 3.2 25.02 ± 4.5 0.002
Cholesterol (mg/dL) 130.2 ± 59.8 152.6 ± 61.9 0.059
Albumin (mg/dL) 4.08 ± 0.6 4.29 ± 0.5 0.054
Hemoglobin (g/dL) 9.3±1.9 11.1±2.4 0.000
EPO dose (UI/w) 9024 ± 4486 6126 ± 5458 0.006
Ferritin (ng/mL) 292.1 (11-3201) 208.2 (8-1950) 0.044
PTHi (pg/mL) 577.3 ± 603 665.9 ± 495.7 0.430
The relevant cytokines are presented in Figure

CONCLUSION: Patients with renal graft nonfunctional have a trend to develop a state of chronic inflammation, expressed in the cytokine profile exposed. The Failed Kidney Graft group showed a lower levels of albumin, cholesterol, hemoglobin, BMI and a higher requirement of EPO, maybe related to inflamation.
