Knowing impact of BKV viremia and CMV infection on pancreas allograft outcome post Simultaneous Pancreas and Kidney transplant (SPK) may improve allograft long-term survival and assist in focusing longitudinal surveillance.

Longitudinal data from 786 SPK recipients from January 2000 to April 2016 were analyzed for the incidence of BK and CMV and post-infection pancreas allograft rejection and survival.

Results: A total of 45 (5.7%) BK viremia and 32 (4.1%) CMV infections occurred during a median follow-up of 4.3 years (incidence rate per 100-person-years = 1.32 for BKV and 0.85 for CMV). Median time to BKV diagnosis was 217 days and for CMV infection was 163 days. Older age (adjusted hazard ratio [aHR]=1.94 for each decade; 95% CI: 1.25, 3.02) and tacrolimus use (aHR=19.3; 95% CI: 1.3, 280) were associated with incidence of BKV but not CMV viremia. A total of 29/45 (64.4%) and 19/32 (59.4%) recipients experienced pancreas graft failure following BKV and CMV viremia, respectively, whereas 9 (20.0%) and 2 (6.3%) experienced pancreas rejection. Kaplan-Meier curve is suggestive of significant loss of pancreas allograft after BK Viremia or CMV infection. Result: Older-age at time of transplant and tacrolimus use are independent predictors of BKV viremia. BKV and CMV infection are associated with inferior pancreas allograft survival.
