*Purpose: Two thymoglobulin (TG) dosages effect on the Tregulatory cell (Treg) and describe the behavior of the co-stimulatory pathways in terms of FIM and gene expression pattern. TG induction reduces acute rejections in kidney transplant and improves patient’s quality adjusted life years in long-term follow-ups, particularly inpatients with higher immunological risk patients. The efficacy of TG in controlling graft rejection is thought to be as a result of its ability to delete various immune cells, especially T cells. The evidence suggest that TG induction may preferentially kill conventional T cells resulting in an increased proportion of Tregs. It is known that T reg co stimulatory pathways as cd28, CTLA-4, ICOS and PD1 may play a role in outcome of the allograft. However, there is not strong evidence which is this role.

*Methods: T reg FOP3 positive cell), cd28, CTLA-4,ICOS, PD1 were evaluated in a cohort of 48 patients in 4 groups: kidney transplant (KT) recipients who will receive induction with standard doses of basiliximab (BX)) . KT with low dose of TG (equal to or less than 3 mg/kg). KT recipients with standard doses of TG (4.5 mg/kg). Control group of donors. Samples were analyzed by flow cytometry, at time 0 8, 30,90 days after surgery. Treg cell FACS, mRNA expression of FOXP3 and co inducible pathways were studied with RT2 Profiler PCR Array considering the genes involved in the processes associated T reg and co inducible pathways.

*Results: Cd4 and T reg cells showed no differences in the groups at time cero. As was expected 8 days after surgery no Cd4 cells were observed in both groups of TG , and a significant difference was observed with BX group. At 30 days both TG groups did not shown differences in cd 4 and Treg cells, BX group has more cd4 positive cells than TG. Both TG groups have more Treg cells than BX (p=0.04). Same result was observed at 90 days. Co inducible molecules showed No differences in all the patients. Gene expression pattern in T reg co inducible molecules showed that the transcriptional patterns modifies depending the therapy.

*Conclusions: Low and standard dosage of TG has the same effect on the Treg cells, the modifications in the expression pattern could explain the differences in the clinical outcome of the allograft.
