Objective: To evaluate whether volume of the remnant kidney based on pre-donation CT and donor age correlates with post-donation 2-year renal function in live kidney donors.

Materials and methods: In this retrospective single-center study, 51 live kidney donors were identified who underwent pre-donation CT and had an estimated glomerular filtration (eGFR) determination at 2 years post donation. Age, retained kidney whole and cortical volumes, and eGFR at 2 years post donation were recorded. Donor eGFR<60 ml/min/1.73m2at 2 years post-donation was used as cut-off value. Pearson correlation coefficients (PCC) evaluated the relationship of age and retained volume/BMI ratio with 2-year eGFR, and logistic regression models for those factors significantly associated with a2-year eGFR<60 were created. Optimal cut-points were identified, and models using these were created to predict eGFR<60 at the end of 2 years.

Results: Retained cortical volume/BMI ratio (CV/BMI) was the normalized volume measurement most strongly associated with the 2-year eGFR (PCC=0.66). CV/BMI and age were associated with 2-year eGFR<60 in a logistic regression model (p=0.02 and <0.01, respectively). The area under the receiver-operator curve for a model that included CV/BMI and age was 0.9. Optimal cut-points to predict eGFR≥60 at 2 years were age<45 years and CV/BMI≥4.5 mL/(kg/m2). If both risk factors were unfavorable (age>45 years and CV/BMI<4.5), the probability of a 2-year eGFR<60 estimated by the model was 92%. If both did not meet the age and BMI thresholds (age<45 years and CV/BMI≥4.5), the probability was 3%. If only one factor was favorable (age<45 years or CV/BMI≥4.5), the probability was 36 or 39%, respectively. (Table 1)

Conclusion: Pre-donation CT CV/BMI ratio of the remnant kidney and age are important factors inpredicting eGFR<60 in donors 2 years post donation and may thus provide important decision support during the live donor selection process.
