The literature on living donor liver transplantation (LDLT) for alcoholic liver disease (ALD) is sparse. We report our experience w/LDLT for ALD at a single, high-volume US Center. Methods: Between 2002-2015, we analyzed recipients who underwent LDLT for ALD and compared this with non-ALD cohort using non parametric tests, Kaplan Meier curves. Results: 136 LDLT; 22 for ALD. For ALD: median age 55.5 yrs, 60% male, MELD 15.7, 73% R lobe grafts; 73% prior drug use, 41% prior tobacco, all had min sobriety 6 mos. 4 patients died; 6 lost grafts. 3 relapses: 1 alcohol, 1 tobacco, 1 lost to follow up. Tables 1 and 2 compare LDLT for ALD vs other indications.

No significant difference in graft fxn, LOS, biliary complications (Table 2), 1-or 5-year patient (Figure 1) or graft survival. (Figure 2) Conclusion: Excellent outcomes, low recidivism w/LDLT for ALD.
