Aims It has been showed that combined liver-kidney normothermic machine perfusion (NMP) is able to better maintain the circuit's biochemical milieu. Nevertheless, whether the combined perfusion is superior to liver perfusion alone in protecting livers from warm ischemia injury during donation after cardiac death (DCD) is unclear. We aimed to test the hypothesis and explored the underlying mechanisms.
Methods The livers from 15 DCD pig donors were subjected to either static cold storage group (Group A), liver alone NMP group (Group B), or the NMP combined liver-kidney group (Group C). All livers were preserved for six hours and reperfused ex vivo to simulate transplantation for two hours.
Results After reperfusion, Group C showed an improvement of the circuit's acid-base and biochemical environment., with the best functional and hepatocellular injury outcomes, followed by Group B, the CS group had the lowest bile production and the worst hepatocellular injury.According to histology, Group A presented with hepatocyte congestion, necrosis, denudated biliary epithelium, and submucosal bile duct necrosis. whereas Group B showed mild injury to the liver parenchyma and biliary architecture, and Group C displayed an almost normal architecture. Furthermore, Group C were also showed the lightest oxidative stress and inflammatory injury among the three groups. Ki-67 staining and apoptosis showed liver and biliary epithelial regeneration in NMP groups.
Conclusion Normothermic perfusion is a beneficial preservation modality that is able to improve metabolic and functional parameters of livers and bile duct, and the combined perfusion of liver-kidney can enhance the effect by maintaining the stability of the internal environment and abolishing oxidative stress injury.
