*Purpose: The use of organs from donors with infection is limited because of the possibility of transmission. Herein we report our experience of transplantation from donors with bacteremia.
*Methods: Retrospective study of patients undergoing kidney or pancreas transplantation at Seoul National University Hospital between July 2007 and November 2019 were performed. We analyzed the outcomes of patients who received transplantation from donors who had bacteremia before transplantation.
*Results: During study period, a total of 74 donors were found to have infection before transplantation. The primary source of infection was respiratory tract in 66 donors, urinary tract in 16 donors, and bloodstream in 14 donors. The most common isolated pathogens from bloodstream were Gram-positive bacteria (10; 6 coagulase-negative Staphylococcus, 3 Enterococcus, and 1 S. viridans), Gram-negative bacteria (4; E. coli, A. baumannii, P. aeruginosa, and M. morganii), and Candida (2). Appropriate antibiotics were used in 42.9% of donors before transplantation and in 35.7% of recipients after transplantation. Three cases of urinary tract infection and 1 case of respiratory infection occurred within one month after transplantation, but none of them were suspected of transmission.
*Conclusions: Using organs from donors with positive blood culture seems to be a safe option with low risk of transmission.
