Background / Purpose: Pancreas is complicated by its blood supply and vascular structure. At present, it is necessary to do the vascular remodeling and reconstruction for the transplantation of the pancreas in the world.In wertern countrys, donor pancreas has been obtained separately, celiac artery was distributed to donor liver, donor pancreas only retained the splenic artery and superior mesenteric artery, so they need the donor internal and external iliac artery and common iliac artery to reconstruct the "Y" type of bypass with pancreatic splenic artery and superior mesenteric artery before the transplantation of pancreas.In China, the donor pancreas may retain the celiac artery and superior mesenteric artery and abdominal aorta patch, but the common hepatic artery was preserved to the donor liver. Therefore, the proximal of the common hepatic artery and the gastroduodenal artery are divided inevitably, and usually its broken ends are anastomosised to reconstruct the head of the pancreas arterial arch in the domestic pancreatic transplantation center. In the practice of clinical pancreas transplantation, we improved the method of vascular repairing for pancreas transplantation,meanwhile, the donor operation time was obviously shortened.

Method:improvement of the surgical method of the artery and vein of the transplanted pancreas graft.Vein:dividing the portal vein in the middle of duodenum ligament. Artery: do not need to reconstruct the broken end of the gastroduodenal artery and the common hepatic artery, only ligation respectively.In addition, we use the celiac artery and superior mesenteric artery and abdominal aorta patch as transplanted pancreatic artery common stem, there is no need to carry out the "Y" type of bypass reconstruction. As a result, a total of no vascular anastomosis reconstruction was performed in the pancreatic repair operation.

Results: Eight cases of pancreas transplantation were performed by this methods, the time required for the repair operation was 2.5-3h, it is obviously lower than the normal control group, that was required 4.5h. After operation, the transplanted pancreas was well active, and no vascular complications occurred.

Conclusion:we can significantly shorten the time of pancreatic repair operation by the method of donor pancreas transplantation without vascular remodeling and reconstruction, furthermore, it greatly reduce the bleeding, thrombosis and other vascular complications in theory.
