Objective
Infectious complication is the leading cause of graft dysfunction and patient loss after renal transplantation (RT). In the early post-transplant phase where acquired immunity is strongly suppressed by the use of immunosuppressive drug to prevent rejection, component of innate immunity plays a significant role in preventing against infection in this immunosuppressive condition after RT. Polymorphism in the component of innate immunity FcgRIIA, FcgRIIIA and C1QA may have impact on infectious complication after RT.Therefore, our study investigated the impact of single nucleotide polymorphism (SNP) of FcgRIIA, FcgRIIIA and C1QA genes on infectious complication after RT.
Methods
82 consecutive patients were enrolled who underwent primary renal transplantation. The SNPs of FcgRIIA (131 H/R), FcgRIIIA(158 F/V) and C1QA (276 A/G) were defined by performing PCR-RFLP method. Those SNPs were analyzed with respect to clinical outcome of infectious complication within 1 months of transplantation i.e. incidence of cytomegalovirus (CMV) infection, Varicella zoster virus(VZV), blood stream infection (BSI),Fungal infection and Urinary tract infection (UTI).
Results
We observed,incidence of UTI was statistically significant (P=0.033*) in patients with F carrier of FcgRIIIA (n=19/38, 50%) than VV homozygous (n=12/44, 27.27%).Similarly HH homozygous of FcgRIIA were tends to be associated with the incidence of UTI (P=0.081) and when we combined FcyRIIA and FcgIIIA genotype for stratification we found the statically significance (P=0.010*) between the two groups i.e. HH genotype of FcyRIIA and F carrier of FcgRIIIA group had more infection (n=15/25, 60%) than that of R carrier of FcgRIIA and VV genotype of FcgRIIIA (n=2/12, 16.66%). We also found that C1QA genotype were tends to be associated with incidence of CMV infection (P=0.053) i.e. 5 of 17 patient (29.41%) in AA homozygous compared to 7 of 65 patient in G carrier (10.76%). We further found no association of this three gene polymorphism with other infectious complication or graft survival.
Conclusion
SNP of these innate immune related gene are associated with the infectious complications following early post RT period which could be important predictor and help to identify high risk group and to decide the intensive prophylactic therapy with antibiotics or antiviral on the suspected individuals before and after RT.
