Background: The mortality of Cryptococcosis remains unchanged among kidney transplant recipients(KTR) over the past two decades, and the prognosis of Cryptococcosis is yet unpredicted. Hence, we conducted this retrospective study to determine the incidence, influential factors and prognosis in KTR diagnosed with Cryptococcosis.
Methods: Retrospective of all KTR who were diagnosed and treated for Cryptococcosis at our institute between January 2010 and June 2016.
Results:Of these 27 KTR, the average period from symptoms to diagnosis was 32±38 days. Cryptococcosis mainly involves the brain (group A, n =7) or the lung (group B, n =7) or both (group C, n = 10). The lung feature in histopathology was nodule (4/7 in group A, 8/10 in group C), while characterized by cough and sputum in group A(6/7), headache alone in group B(7/7), headache and fever in group C(8/10) in terms of lung manifestation. The mortality was 0 in group A, 1 in group B and I in group C.
Conclusion: The incidence of Cryptococcosis infection is relatively low, nonetheless, its diagnosis is often delayed. The mortality of Cryptococcosis can be lowered by timely and effective therapy. The identification of Cryptococcosis is a tough issue, which asks us to be cautious of cerebrospinal fluid, skin changes in presence of nonspecific respiratory symptoms, or inversely. This study highlights the urgence for further research to improve the primitive detection and management of a potential infectious disease among KTR.
