*Purpose: The optimal strategy to safely transition a pediatric kidney transplant recipient to an independent adult kidney transplant recipient is vital to ensure compliance and maintain renal allograft function. We describe the multi disciplinary transition program that we have employed for pediatric kidney transplant patients who are approaching adulthood at our center.

*Methods: This is a descriptive analysis of the transitioning program that has been employed at our center since 2012. We studied how many patients enrolled in this program have missed clinic appointments and routine laboratory appointments (non compliance) leading to graft failure after transitioning to the adult transplant nephrology clinic.

*Results: Since 2012 we have transitioned 19 pediatric transplant recipients (Average age 19.5 years, 9 males, 10 females) to the adult transplant nephrology clinic using a multi disciplinary team of transplant physicians, social workers, post transplant coordinators and psychologists. The exact time of transition is dependent on the readiness of the patients, their families and the medical care team. Our unique approach is characterized by combined clinic visits between pediatric and adult transplant care providers. Only 4 patients have lost the allografts due to non-compliance and rejection.

*Conclusions: Though there are no comparative study groups in our patient population, our analysis shows pediatric kidney transplant recipients require a multidisciplinary care team to ensure a safe transition to adulthood to help in maintenance of allograft function.
