*Purpose: To assess complication and survival after performance of kidney transplantation in a resource poor setting in Southeast Nigeria
*Methods: A retrospective review of transplant registers of the first 10 patients who underwent kidney transplantation at Federal Medical Centre Umuahia Southeast Nigeria over a three-year period. Socio-demographic characteristics, aetiology of ESRD, time on dialysis, HLA match with donor, Donor kidney function, immunosuppressive regimen, cold ischaemic time, other relevant data were collated. Also the survival of patients and the kidney functions were assessed.
*Results: A total of 10 kidney transplantations were reviewed. All recipients were males with median age of 38 years (16-60 years). The HLA mis-matches with donors included 4/6 in 5 (50%), 3/6 in 4 (20%) and 1/6 in 1(50%). Median duration of dialysis in recipients is 8 months (1-25). Induction regimen was the anti-thymocyte globulin while maintenance regimen were tacrolimus, mycophenolate mofetil and prednisolone in all patients. Some short term complication recorded included urinary tract infection (60%), urinary catheter-related discomfort (20%), delayed graft function (20%) and Antibody mediated acute rejection (10%). Some long term complications included sepsis (30%), post-transplant erythrocytosis (10%) and graft dysfunction (20%).One year graft outcome was 80% while patient outcome was 100%. Three year graft outcome was 70% with patient outcome of 80%.
*Conclusions: The one year and three year patient and graft outcome is comparable to what is obtainable in developed countries. Post transplant complications were considerable.
