Background: Current knowledge of the impact of facial vascularized composite allograft (VCA) procurement on the transplantation outcomes of the concomitantly recovered life-saving organs is limited to isolated case reports and short-term results. Here we report the largest series to date of facial allograft donor surgery experience and long-term outcomes of the concomitantly recovered solid organs and their recipients.

Methods: We performed facial VCA procurements in 7 organ donors between April 2009 and October 2014. We identified the population of subjects who received solid organs from these 7 donors using the Scientific Registry of Transplant Recipients. We retrospectively reviewed operative characteristics, intra-operative parameters, and postoperative outcomes.

Results: Six of seven donor surgeries were performed at outside institutions, all on brain-dead donors. Mean operative duration for facial VCA recovery was 4.9 hours (range 4 – 7 hours). A total of 23 solid organs were recovered and transplanted into 23 recipients

. Survival rates for kidney and liver recipients were 100% at a median follow-up of 36 months. Graft survival rates for kidneys and livers were 10/11 (91%) and 7/7 (100%), respectively. An intestine recipient was on oral feeds at 6 months. The pancreatic (n=1) and single lung (n=1) recipients were alive and their grafts functioning at 6-months follow-up, respectively. No data has been reported for one heart recipient.

Conclusions: Our protocol for facial VCA procurement does not appear to adversely affect the outcomes of transplant recipients of concomitantly recovered solid organ allografts.
