Purpose: To determine the effect of regional variability in wait time
on survival of patients with hepatocellular carcinoma(HCC) listed for deceased donor liver transplantation(DDLT).
Methods: Patients age>18 with HCC exception points in the OPTN database from 2/27/2002-9/15/2015. Outcomes were calculated and compared between UNOS regions.
Results: 17,023 HCC patients were identified. Median wait time varied by region. One-year transplant rate(region 1,5,9<2,4,7<6,8<3,10,11,p<0.05) and 1-year post-DDLT survival(range:86-92%,p<0.001) varied significantly by region. Cumulative incidence(CI) of post-listing 1-year mortality was significantly higher in long wait time regions 5, 7, 9.[table 1]
Conclusion:
Transplant rate and 1-year post-DDLT survival varies by region. The CI of death is higher in long wait time regions.
