Impact of Renal Transplant on the Outcomes of Breast Cancer Surgery

*Purpose: Breast Carcinoma has the highest incidence of any cancer in adult females. The impact of a prior kidney transplant (KT) on breast cancer has not been catalogued in the literature. We aim to evaluate the influence of a history of KT on the outcomes of breast cancer in women at transplant and non-transplant centers.

*Methods: A retrospective analysis was conducted using Nationwide Inpatient Sample (NIS) data between 2005 and 2014. Study population included adult female patients with history of kidney transplant who underwent mastectomy or lumpectomy to treat a malignant breast cancer. Weighted multivariate logistic and linear regression models were used to compare outcomes at transplant and non-transplant centers.

*Results: 75 women that met the inclusion criteria for the study. Patients with prior KT and breast cancer had higher mortality index. On univariate analysis, we noted that these patients have significantly longer length of stay (p=0.03), higher expenditure (p=0.034), higher rates of complications (p=0.026) and require more RBC transfusions (p=0.036).

Weighted multivariate analyses highlight that rates of complication (p=0.009), length of stay (p=0.001), and total expenditures are lower (p=0.001) at transplant centers compared to non-transplant centers.

*Conclusions: History of kidney transplant has a statistically significant impact on the outcomes of patients undergoing mastectomy or lumpectomy for breast cancer treatment. These patients suffer more post-operative adverse events compared to the non-transplant cohort. However, previous KT patients benefit from treatment of their breast cancer at transplant centers compared to other hospitals and report superior outcomes.
