Introduction:Resistin has a pathogenic role in the development and progression of atherosclerosis. This study was evaluated the association between serum resistin levels and future acute kidney injury events in patients with kidney transplantation (KT).

Materials and Methods: A total of 68 KT patients were enrolled in this study. The primary end point was the acute kidney injury according to KDIGO criteria.

Results: During a median 65-month follow-up, 30 acute kidney injury events occurred. Serum resistin level was positively associated with BUN, creatinine, while negatively associated with GFR. KT patients with acute kidney injury events have higher BUN, creatinine, and higher serum resistin levels, but lower GFR compared to subjects without acute kidney injury events. The Kaplan–Meier analysis showed that the cumulative incidence of the acute kidney injury events in the high resistin group (median resistin concentration of ≥ 12.72 ng/mL) was greater than that in the low resistin group. By multivariate Cox analysis after adjusted age, gender, diabetes, hypertension showed that GFR and serum resistin levels was independently associated with acute kidney injury events in KT patients.

Univariate Multivariate
Hazard Ratio (95% CI) P value Hazard Ratio (95% CI) P value
Resistin, 1 ng/mL 1.043 (1.019-1.068) < 0.001* 1.030 (1.001-1.060) 0.046*

Age, 1 year 1.009 (0.972-1.048) 0.624 1.008 (0.966-1.053) 0.704
Male gender 1.028 (0.718-1.471) 0.881 1.191 (0.800-1.771) 0.389
Diabetes mellitus 1.181 (0.562-2.483) 0.660 1.188 (0.511-2.762) 0.689
Hypertension 1.772 (0.860-3.650) 0.121 1.319 (0.570-3.048) 0.518

Conclusion:Serum resistin level is a biomarker for future acute kidney injury events in KT patients.
