Introduction : Focal segmental glomerular sclerosis (FSGS) glomerulonephritis can recur in renal transplants, and can potentially lead to graft loss. The rate of graft loss in second renal transplant ,following failure of the first graft due to recurrence is unclear.
Methods: A retrospective review of the UNOS/OPTN database from 2004 to 2014 was performed. Patients were included for analysis if their first graft failed due to biopsy-confirmed recurrent focal segmental glomerular sclerosis. We then followed their course and looked at incidence of graft loss due recurrence of FSGS in the second transplanted kidney. Both adult and paediatric transplants were included.
Results : 337 recipients had failure of first graft due to recurrence of FSGS .Of these 337 recipients,73 had second renal transplant . 40 % recipients had living donor kidney transplant .14% of the these recipients were in paediatric age group . Of the 73 re-transplants ,12 grafts were lost due to recurrence of FSGS in second renal allograft (16.43 %).1 graft failed within first 6 months , 3 grafts failed between 6months to 1 year and 9 failed between 1 year to 5 years.3/12 (25%) graft loss was in living donor recipients and 9/12 ( 75%) in deceased donor recipients.3/12 (25%) graft loss was in paediatric age group recipients .
Conclusion :FSGS recurrence leading to graft loss does not necessarily affect all re-transplants following loss of the first graft to FSGS recurrence. Patients who experience first graft failure due to recurrent FSGS should not be precluded from receiving a second transplant.We will need to identify risk factors and interventions to prevent statistically significant graft loss due to recurrence of FSGS in recipients of second kidney transplant whose first graft failed due to recurrence of FSGS.
