Forty-Six Consecutive Cases of Pure Live Donor Right or Extended Right Hepatectomy: Single Surgeon Initial 1-Year Experience.

The pure laparoscopic live donor hepatectomy has been performed in several experienced transplant centers. However, short and long-term surgical outcomes are not yet to be determined. The aim of this study is to present the initial 1-year experience of pure laparoscopic live donor hepatectomy performed by a single surgeon. Data from 46 consecutive patients who underwent pure laparoscopic live donor right or extended right hepatectomy which was performed by single surgeon at Seoul National University Hospital from November 2015 to November 2016 were retrospectively analyzed. There were no specific selection criteria. Cases with separate right anterior and posterior portal veins or any other variations of bile ducts were not excluded. Right hemihepatectomy was performed in 42 patients (91.3%) and extended right hemihepatectomy, that is containing the middle hepatic vein was performed in 4 patients (8.7%). There were no intraoperative complications with no transfusion. There was no reoperation and there was no conversion. Mean operative time was 339.8 minutes and mean estimated blood loss was 400.2 ml. Mean hospital stay was 8.2 days. There were 4 cases (7.8%) of postoperative complications including 2 cases of biliary leakage and 2 cases of wound problem. Among 2 cases of biliary leakage, 1 case was resolved spontaneously and the other was treated by endoscopic retrograde biliary drainage. In conclusion, pure laparoscopic live donor hepatectomy is a feasible procedure. Further experience including long-term outcome would be needed to conclusively standardize a feasible procedure.
