*Purpose: To compare the long-term outcomes of older (50-65 yr) diabetic patients who received Simultaneous Kidney pancreas transplantation (SKP) vs. living donor (LD) kidney transplants.
*Methods: This is a retrospective single-center study from July 2003 to March 2021. LD recipients were 50-65 years old with primary kidney disease due to Diabetes Mellitus.
*Results: There were 107(44%) subjects who received SKP, and 135 (56%) received LD transplants. Mean follow-up was 7.4 (4.4) years, and median(IQR) follow up 7.03 (4.1,11.4) years. Table 1 describes recipient and donor characteristics. The mean age of the recipients in the SKP group was 56 vs. 58 years in the LD group. Duration of diabetes was 32 years in SKP vs. 23 in LD. Length of dialysis and number of preemptive transplants were similar. The donors in the SKP group were younger (26.5 vs. 42, p <0.001) and more likely to be male (71% vs 33%, <0.001). Serum creatinine at 4, 12, 24, and 36 months was similar. Incidence of kidney rejection by 1 year (for cause and protocol biopsy, including borderline rejection) and BKV infection was similar. Estimated 3-year death censored kidney graft survival was (96% vs. 99.1%) for SKP vs. LD. There was 18 (17%) pancreas graft loss within 1 year in the SKP group. Death censored kidney (log-rank 0.18) and patient survival (log-rank 0.48) were similar.
*Conclusions: Our study suggests favorable outcomes in older SKP recipients when compared to recipients of living donor kidneys.
