Facial Allotransplantation: An Early Look at OPTN Data

Craniofacial Vascularized Composite Allograft (VCA) transplantation is a developing area in the field of transplantation. Thirteen craniofacial transplants have been performed in the US since 2008. Follow-up time for patient and graft status ranged from 0.1 years to 8.3 years post-transplant (median=3.7 yrs; IQR: 2.0-6.3). Of the 13 transplants, 9 (69.2%) were male. Recipients' age ranged from 21-59, and 5 (38.5%) were 18-34 years, 4 (30.8%) 35-44 years, 2 (15.4%) 45-54 years, and 2 (15.4%) >55 years old. 13 (100.0%) were white. Eight (61.5%) required a transplant because of trauma and five (38.5%) because of burns/explosions. As of November 10, 2017, no craniofacial VCA recipients in the US have died after transplant, and all recipients have intact grafts. Of recipients with reported data, none have developed new onset diabetes or metabolic complications. Of nine with reported data, three (33.3%) had infectious complications, one (11.1%) had other complications, and eight (88.9%) had at least one acute rejection episode. The OPTN currently is collecting functional outcomes data on all craniofacial transplants and plans to report these results in spring 2018.
