Primary non function(PNF) is a severe complication after liver transplantation which causes recipient death or re-transplantation in a short term. PNF occurs more common in liver transplants using donation after death (DCD) organs. We aimed to construct a reliable tool to predict the occurrence of PNF in DCD transplants. The clinical data of recipients from September 2001 to February 2016 in SRTR database were reviewed. 3278 DCD transplants fulfilled the criteria for analysis, with a PNF rate of 4.9%, which was higher than that (3.1%) in donation after brain death (DBD) transplants. Multivariate logistic regression analysis identified five independent factors of PNF, including candidate renal insufficiency, candidate age, warm ischemic time, use of ventilator, cold ischemic time. Based on which a nomogram was constructed to predict PNF. The nomogram showed an area under the operating characteristic curve (AUC) of 0.68 (0.61-0.76) in the training set, and 0.76 (0.62-0.89) in the validation set, which were superior to the model for end-stage liver disease (MELD) score, MELD-Na score and donor risk index. The calibration curves in two sets both showed a good agreement between predicted PNF and actually observed PNF. Herein, we developed and validated a robust nomogram that might be used to avoid futile transplantation and help organ allocation.
