Purpose

To compare local recurrence rates (LRR), tumor response rates, and dose delivered between standard endhole catheters (EC) versus anti-reflux catheters (AR) in patients who underwent Drug-eluting Bead Trans-arterial Chemoembolization (DEB-TACE) with solitary LIRADS 5 lesions (HCC) at a single institution.

Materials and Methods

In this retrospective study include all treatment naive cirrhotic patients with solitary LIRADS 5 tumors less than 6.5 cm who underwent DEB-TACE were evaluated. In this single-center study, a total of 83 patients were initially treated over a two year period by either the use of EC (n=69) or AR (n=14) using DEB-TACE (100-300 [micro]m beads). LRR and Complete Response Rate (CRR with mRECIST) at 1 month were evaluated using the two Pearson Chi-square Test and Fisher Exact Test to assess statistical significance.

Results

EH and AR cohorts were similar tumor size, p=0.78. Rate of LRR at one month was significantly lower in patients treated with AR (14% vs. 43%, p<.05). Tumors treated with the AR infusion system also achieved a significantly higher CRR (79% vs. 56%, p<.05). There was a trend of lower dosage requirement for patients treated with AR (p=.08).

Conclusions

Treating solitary LIRADS 5 lesion with AR was associated with lower rates of recurrence and higher complete response rate. The AR system also showed a trend toward lower dosage.
