Kidney function, as measured by GFR, at 6 months post-kidney transplant is a powerful predictor of long-term post-transplant outcomes. This study sought to understand the relationship between kidney donor profile index (KDPI) of the donor and GFR at six months in the recipient.
All adult deceased donor kidney recipients with six months of graft survival and a six month follow-up creatinine transplanted between January 1, 2005 and December 31, 2012 were included in the analysis. 74,387 recipients had 6 months of graft survival. Of these recipients, 96.9% (n=72,046) had a follow-up creatinine at six months. eGFR was calculated using the CKD EPI equation.
A linear relationship existed between median eGFR at six months and KDPI of the donor.
Linear regression analysis showed for every 10% increase in KDPI the median eGFR dropped 3.1 ml/min/1.72m2.

Finally, we determined the eGFR distribution at six months based on the allocation KDPI groups (0-20%, 21-85% and 86-100%).
The median eGFR was 68.7, 55.3 and 42.5 ml/min/1.72m2 respectively.
KDPI is a strong predictor of graft function at six months and is an important consideration when determining recipients of high KDPI organs.
