Quality of Life of Brazilian Kidney Transplant Patients: A Sub-Project of ADHERE Brazil Multicenter Study

*Purpose: While the quality of life (QoL) is known to improve after kidney transplant (KT), epidemiologic data are limited in Brazil. We aimed to describe the QoL of Brazilian KT patients according to age, gender, and a Brazilian reference cohort.
*Methods: Secondary data analysis of the ADHERE BRAZIL cross-sectional study including 20 kidney transplant centers and 1,105 KT patients (data collection: Dec/15 to Jun/17). Patients were proportionally and randomly selected using a multistage sampling method, considering the geographical region (South/Southeast and North/Northeast/Mid-West) and transplant activity of the centers. QoL was assessed by the WHOQOL-Bref (general QoL and four domains (physical, psychological, social relationships, and environmental). Data was descriptive and compared by GEE.
*Results: We studied 1,105 patients, 58.5% males, with a mean age of 47.6±12.6 years. The general QoL scores were 81±15.1, 58.6±11.6 for physical, 65.5±11.4 for psychological, 68.3±17.1 for social relationships, and 64.2±13.3 environmental domain. For men, higher QoL scores were reported in psychological (OR 2.62, CI:1.29 ‑‑3.95, p<0.0001), social (OR 3.21, CI:1.2 ‑‑ 5.23, p=0.002), and environmental (OR 3.79, 2.23 ‑‑ 5.35, p<0.0001) domains. Younger patients (18-44 years) had higher scores in the psychological (OR -0.37, CI: -0.80 ‑‑ -0.24, p<0.001 and OR -0.28, CI: -0.60 ‑‑ 0.18, p<0.001) and social (OR -0.28, CI: -0.78 ̶ -0.18, p=0.002 and OR -0.14, CI: -0.23 ̶ -0.10, p<0.0001) domains than older ones (45-59 years and >60 years, respectively). Compared to a general Brazilian reference population sample, KT patients scored similarly in the general and physical domain, worse in social relationships but better in the environmental domain.
*Conclusions: In this larger sample of KT patients, the most affected domain is the physical, and the best scores are attained in the social relationships domain. There are some subgroups of lower QoL: women, age >45 years. Highlighting the good results of a kidney transplant as a treatment, the QoL of Brazilian kidney transplant patients was closer to the general population but in social relationships.
