*Purpose: Currently, the follow-up of KT in the clinic remains the creatinine levels, this is a highly specific, however, without sensitivity to determine diagnosis of rejection in the renal allograft. Objective: To evaluate the proportion and Median Fluorescence Intensity (MFI) of the Co-stimulation patways in T cells (CD3 +, CD4 +, CD25 +, FOXP3 +, CD279 +, ICOS +) in peripheral blood of patients with renal transplants with rejection.
*Methods: Venopuncture was performed on patients with KT, both rejection as well non rejection. the panel of antibodies used was constructed with CD3+, CD4+, CD25+, FOXP3+, CD279+ (PD-1) and CD278+ (ICOS). The study group was composed of 21 patients, without rejection, and 13 patients with acute rejection diagnosed by biopsy. This protocol was accepted by the research and ethics committee. All participants voluntarily agreed to participate prior to signing the consent. The experimental part was performed in the 3-lane FACSAria Flow-Cell Sorter Cytometer, BD, the results were analyzed in the Infinicyt Cytognos version 2.0.
*Results: Peripheral mononuclear cells cells were obtained by density separation. The values of the MFI for PD1 in the rejection group was 229 SD 204.8, the control group of patients with normal KT of 863.1 DS 584.1; In the contrast of both groups using a non-parametric Mann-Whitney statistic, it shows a p = 0.001 in a two-way analysis (two tails). The values of the MFI for ICOS in the rejection group was 4065 SD 2969, the control group with normal KT of 1747 DS 1171; In the contrast of both groups using a non-parametric Mann-Whitney statistic, it shows a p = 0.041 in a two-way analysis (two tails).
*Conclusions: Negative co-stimulation of PD1 has been shown in multiple mouse models. These results, in patients, are totally consistent from the immunological point of view, which demonstrates the power of costimulation pathways to demonstrate a difference between the rejection and control groups. We firmly believe that the use of flow cytometry as a tool in the clinical setting of kidney transplantation will soon have to be taken into account.
