*Purpose: simplifyKDPI is an iteratively-designed patient-engaged mobile educational animated video program developed with guidance from patients, providers and Self-efficacy and Multimedia Learning Theories to increase knowledge and self-efficacy of kidney transplant candidates about KDPI.
*Methods: Kidney transplant candidates undergoing transplant evaluation at Erie County Medical Center between May-September 2018 were shown the 2-minute video intervention. Knowledge, self-efficacy and willingness to consider accepting high KDPI offers were assessed immediately before and after the video.
*Results: The cohort consisted of 23 patients. Mean KDPI knowledge scores were significantly increased from pre-intervention (5.7±1.8) to post-intervention (7.2±1.0); (Mean difference (MD) 1.5±1.4, P<0.001). Differences remained significant for those with: Age ≥ 58 years (n=12, MD 1.0±1.4, P=0.0323); less than college education (n= 16, MD 1.5±1.3, P=0.0004), on dialysis (n= 11, MD 1.7±1.3, P=0.0409), having low technology access (n=12, MD 1.5±1.5, P=0.0042) and low literacy/numeracy (n=11,MD 1.8±1.9, P=0.0096). Self-efficacy scores also significantly increased pre and post video (MD 1.2±1.3, P=0.0002). There were no significant differences in the willingness of patients to accept high KDPI kidneys overall or those with age ≥58 (MD -0.1±1.8, P=0.73 and MD -0.2±1.8, P=0.75 respectively)
*Conclusions: simplifyKDPI video is associated with significant increases in knowledge and self-efficacy even in patients with low literacy levels and low access to technology. However, no significant differences were observed in willingness to accept these offers.
