*Purpose: Infection is a major cause of morbidity and mortality after organ transplantation and pneumonia is one of the most frequently encountered infections.

*Methods: We conducted a retrospective single-center cohort study of liver transplant (tx) recipients from 2009-2017 to determine the risk factors for post-transplant pneumonia and the impact it had on outcomes.

*Results: 97/513 (19%) patients had pneumonia in the 1^st post-transplant month and 132/513 (26%) in the first year. Patients with underlying COPD, higher MELD score at tx, diabetes mellitus, in-hospital at time of tx, hemodialysis pre-tx, tobacco use history, and those with higher number of ventilator days were at increased risk for post-tx pneumonia (p≤0.05). Pneumonia within 1 month post-tx (Fig 1) and within 1 year post-tx (Fig 2) significantly impacted long-term mortality.

*Conclusions: Pneumonia within 1 year of liver tx may signal recipients that are at a higher risk for post-tx mortality.
