PURPOSE: OPTN data on HCC recurrence is increasingly utilized for a variety of studies; however, the accuracy of this data has not been examined. We hypothesized that OPTN HCC recurrence is underreported when compared to discrete transplant center data.
METHODS: Patients undergoing liver transplantation with HCC exception points from February 27, 2002 to December 31, 2011 at a single institution were studied for HCC recurrence (gold standard) and compared to corresponding patients within the OPTN database with reported HCC recurrence.
RESULTS: 293 patients underwent liver transplantation for HCC, of which 34 were identified with recurrence. OPTN data failed to identify 8 of these recurrences and incorrectly classified one patient with a recurrence. OPTN data had a sensitivity of 76.5% and specificity of 99.6% for HCC recurrence.

CONCLUSIONS: Although documentation of recurrent HCC in OPTN data is indicative of a true HCC recurrence, OPTN data alone fail to identify a substantial number of true cases of HCC recurrence. Validation of these findings among other transplant centers may have implications for transplant policy and studies of HCC recurrence.
