Background.
Kidney transplantation is an effective treatment option for patients with end stage renal disease (ESRD). Although there is a trend to decrease in overseas transplantation and increase kidney transplantation in Azerbaijan, malfunctioning of the local programs makes patients to go for transplantation in outside transplant centres in worldwide. In this study, we aimed to investigate the results of kidney transplantation in Azerbaijani patients who get overseas organ transplantation.
Methods: We retrospectively reviewed patients' charts to access clinical outcomes of kidney transplanted patients in the outside of Azerbaijanduring a period of 18 years. This is a quantitative, cross-sectional, descriptive study based on data collected through documentary analysis and systemic research. The datawas obtained from local nephrology center.
Results. We studied 300 patients' data in details.While 199 patients (66.3%) were male, 101 patients (33.6%) were female. Mean age of the patients were 43+/-11.3 years. Only 25 patients (8.3 %) were transplanted in Azerbaijan. Waste majority of them, 275 patients (91.7%) underwent kidney transplantation in outside of country. Among them 209 transplantations (69.6%) was done in Iran Islamic Republic, 19 (6.3%) in Georgia, 15 (5%) in Turkey, 6 (2%) in Russian Federation, 7 (2.3%) in Pakistan, 4 (1, 3) in Tajikistan. Induction immunosuppressive therapy was given only in 23/ 300 patients. Overall one year graft and patient survival were 91% and 95% respectively. The episodes of acute rejection had occurred in 26/300 patients. Complications were mainly infectious, which 23 patients had got potentially life-threatening one. 13 patients manifested with septic shock and died.
Conclusions: Overall, patient and graft survival were not satisfactory after overseas kidney transplantation and its rates differ from country to country. Although, improper follow up was a main factor, other problems like incomplete preoperative preparation, states of donor organ quality had great impact on postoperative complication and graft survival.
