Previous studies of the United States Kidney Transplant Database revealed that the median estimated GFR (eGFR) has improved over the last decade for living donor recipients. Living donor transplant provides a unique opportunity to understand improvements in the eGFR over time by comparing the eGFR achieved by the recipient at six months compared to the follow-up eGFR of the donor six months after donation and determine if the recipient eGFR is approaching the donor eGFR after donation. In order to study this, all adult living donor recipients performed from January 1, 2000 until August 31, 2013 were linked to the data from their donors. Cases with both a follow-up recipient and donor creatinine at six months were selected for analysis. Using the CKD EPI equation, the eGFR for the donor and recipient at six months after transplant were determined. 80,333 adult living donor transplants occurred during the study period. Of those, 43,662 cases had follow-up creatinines at six months for both the donor and the recipient and made up the study population. The median and mean difference in eGFR between the donor and recipient six months after transplant for the population was 4.80 mL/min/1.73m2 and 5.15 mL/min/1.73m2 respectively (donor eGFR minus recipient eGFR). The difference in eGFR varies significantly by the gender match of the donor to the recipient.

Figure one shows that the difference in eGFR between the donor and recipient and the achieve eGFR of the recipient improved during the 14 years of observation mainly between the years of 2000 and 2007. After 2007 the difference and achieve eGFR of recipient remained relatively stable. This analysis indicates that in the first half of the last decade eGFR of the recipient relative to the donor improved. Understanding the factors that resulted in this improvement may provide insight into better management of recipients.
