Background Recurrent IgA nephropathy is a common complication after kidney transplantation. How to treat recipients with recurrent IgA nephropathy is still controversial. There are two major regimens for recurrent IgA nephropathy. One is supportive regimen with ACEI/ARBs, and the other is supportive regimen plus corticosteroid. We retrospectively reviewed the kidney transplant recipients with recurrent IgA nephropathy in our transplant center from January 2000 to December 2014, with the aim to compare efficacy of two different regimens.

Methods Totally 163 recipients with recurrent IgA nephropathy were diagnosed by graft biopsy after kidney transplants. 105 recipients received supportive only regimen with ACEI or ARBs, the other 58 recipients received supportive regimen plus corticosteroid. Corticosteroid doses was as following: Prednisone 0.5mg/kg/d for 2 months, then reduced by 5-10 mg/d per month until finally tapered to 5 mg/d at 6 month. The serum creatinine levels, proteinuia, hematuria, infection, cardiovascular disease, graft loss and patient death were all recorded during the follow up.

Results the proteinuria levels at 1-year was significantly lower in supportive plus corticosteroid group compared to supportive only group(1.16±0.32 vs.1.53±0.43 g/d, p<0.001).
