The Safety and Efficacy of Suspended Gasless Laparoscopy Living-Donor Nephrectomy in Renal Transplantation.

BACKGROUNDS: To develop a minimally invasive surgery for the kidney transplant donor and get the no CO2 pressure condition harvested kidney, we have developed suspended gasless laparoscopy Living-donor nephrectomy (SGLLDN).

MATERIALS AND METHODS: The safety and efficacy were investigated between 38 cases conventional open live donor nephrectomy (OLDN) and 166 cases SGLLDN from May 2004 to Dec 2014. All the donors had got the ethic committee council board approving.

ESULTS: We investigated the procedures of the operations. SGLLDN were performed in 166 cases while two cases converted to open surgery. There were 164 cases under analysis in the SGLLDN group. The operation time of SGLLDN and OLDN was 90±15 min vs. 110±12 min (p<0.05). The incision length was 8.0±1.5cm vs. 19.5±3.5cm in SGLLDN and OLDN (p<0.01). The blood loss during operation was 90±31 vs. 120±25 ml in SGLLDN and OLDN, respectively(p<0.05) and postoperative hospital stay was 4.0±1.2 days vs. 9.0±1.1 days in SGLLDN and OLDN (p<0.01).The warm ischemia time was 95±10 s in SGLLDN vs. 90±8 s in OLDN (p>0.05), and there were no statistic difference in post operative renal function recover and gastrointestinal function recovery to normal level in both groups. There were no statistic differences in operative or postoperative complications in both groups.

CONCLUSIONS: SGLLDN could be advantageous for healthy kidney donors since it is minimally invasive, smaller incision, less bleeding, shorter operation time, shorter postoperative hospital stay and rapid recovery.
