The purpose of this analysis was to provide information regarding the use and survival benefit of kidney transplantation from a hepatitis C positive deceased donor in the current era.

Using data from the SRTR we identified n =12,841 wait-list patients who agreed to receive a hepatitis C positive deceased donor kidney transplant between 2000-2010. The survival benefit of transplantation with a Hepatitis C positive donor was determined using a multivariate non-proportional hazards analysis with transplantation treated as a time dependent covariate to account for the fact that patients switched treatment from dialysis to transplantation at different times, with follow up censored at the time of living donor transplantation, transplantation from a hepatitis C negative donor or permanent removal from the waiting list. Similar analyses were performed to determine the survival benefit after transplantation from a Hepatitis C negative donor in this cohort.

The table shows that Hepatitis C positive donor transplantation was not associated with a survival benefit in HCV negative recipients and only HCV positive recipients > 50 years of age had a lower risk of death after HCV donor transplantation.

We conclude that the benefit of HCV positive donor transplantation is limited to HCV + positive recipients > 50 years of age.
