Renal Graft Survival among Primary, Re-Transplant, and Kidney after Solid Organ Transplantation: A Single Center Experience

Introduction: With improved patient outcomes after renal transplants, repeat renal and kidney after solid organs are increasingly being done. Because of paucity of outcomes data in these groups when compared to primary transplants, we conducted this retrospective analysis to study their differential outcomes.
Methodology: Registry data for kidney transplantations at UPMC from Jan 2000- Dec 2011 was used. We compared death censored renal graft survival among the three groups using Cox regression analyses, adjusting for recipient/donor age, sex, ethnicity, donor type (living/DBD/DCD), CIT, DGF and rejection at 6 and 12 months.
Results: The details of subjects in each group are summarized in table 1. Median follow up time was 54 months. The 1, 3 and 5 year graft survival rates were 93.8%, 85.7%, 79.4% among primary, 89.8%, 80.8%, 76.2% among re-transplant and 93.2%, 86.9%, 80.7% among other solid organ transplant recipients, respectively. We observed no significant difference in unadjusted actuarial (p=0.24) or death-censored graft survival (p=0.418) between the 3 groups. This finding was confirmed on multivariate analysis adjusting for significant co-variants.
Conclusion: In our single center study, we found that the renal transplant graft survival after primary, repeat and renal transplant after other solid organ transplants were similar.
