*Purpose: Patients with ESRD or advanced CKD have higher incidence of coronary artery disease. We report coronary angiography findings in asymptomatic Pre Kidney transplant recipients with negative stress test but with findings of coronary or vascular calcification on CT.

*Methods: This is a retrospective single center study of 72 Pre Kidney transplant recipients who underwent coronary angiography between 2010 to 2020. All patients had a negative stress test. Coronary angiography was done based on aortoiliac or coronary calcification on CT. Mean age was 60 years in both groups. Demographics/results are listed in table1.

*Results: 17 patients (23%) had a 70% stenosis in one or more coronary vessel. 10 patients (13.85%) underwent single or multiple vessels stent placement and one underwent CABG. 6 patients had significant distal disease or vessels were too small for intervention. 13/17 (76%) were diabetic. All patients underwent successful kidney transplantation. Post Operative hypotension was more common in non CAD group 9 as compared to 3 in CAD group. 5 non CAD recipients had post op Atrial fibrillation vs 1 in CAD group.

*Conclusions: Aortoiliac or Coronary vascular disease is a predictor of significant coronary artery disease. Pre Kidney transplant recipients with negative stress test but significant vascular calcification should be considered to undergo coronary angiography before transplantation.
