Aim: The aim of our study was to evaluate whether preserved kidney volume correlates with and can predict donor renal function at 2-years post-donation using the CKD-EPI eGFR equation.

Methods: Donors with CT volumes were evaluated for pre-donation volume and post-donation function from 2008-2012. Preserved Kidney Volume (PKV) was adjusted (aPKV) to Body Surface Area.

Results: A total of 208 donors were analyzed. Mean age 39.4+/-10.7; Males 36.2%; White Race 91.9%; Mean pre-donation CKD_EPI eGFR 100.5+/-16.5 ml.min. Mean Total Kidney Volume 388+/-86 ml, median aPKV 180 (159-205)ml. Further sub-analysis included the aPKV divided into tertiles (low/medium/high). For dichotomous analysis, a 2 years eGFR CKD-EPI of <60ml/min was chosen as the cut off. Pre-donation eGFR (β Coef. 0.344), age (β Coef.-0.318) and aPKV (β Coef. 0.11) were found to be independent predictors of 2yr eGFR (p<0.001).

Mean 2yr eGFR was 57.9±12, 65±16 and 73±17 between tertile groups (p<0.05). The proportions of donors renal function at 2-years with eGFR (CKD-EPI) >90, 60-89, 45-59 and < 45ml/min/1.73m2 were 10.0%, 45.6%, 37.1% and 7.2% respectively. The odds ratio of having a 2yr post-donation eGFR <60 with a cutoff of median aPKV of 194ml was 5.8 (95%CI 2.9-11.6; p<0.0001); eGFR <60 in the lowest aPKV tertile was 3.51 (95%CI 1.9-6.4; p <0.0001) ; eGFR <60in the highest aPKV tertile was was 0.23 (95%CI 0.12-0.44; p<0.0001). ROC and AUC for aPKV and eGFR (CKD-EPI) of <60 were 0.763 (CI 0.69-0.82) p=0.005.The group with eGFR (CKD-EPI) < 60ml/min had a significantly lower aPKV of 165+/-29 as compared to the group who had 2 years eGFR (CKD-EPI) > 60 who had mean aPKV of 194+/-30.

Conclusions: Remaining kidney volume should be taken into account when selecting kidneys from donors or those with marginal acceptable function to avoid leaving them with a lower functioning kidney.
