Pancreas Transplant Recipients with End-Stage Renal Disease Benefit from Kidney Transplantation

Background: End-stage renal disease (ESRD) has been shown after pancreas transplant alone (PTA), however, the benefit of kidney transplantation (KT) in PTA recipients with ESRD remains unknown. We aimed to evaluate kidney transplant survival benefit in previous pancreatic transplant recipients (KAP) compared to primary isolated (KA1) or repeat isolated kidney (KA2) transplant.
Methods: All elective kidney transplants performed for the period 1995-2013 recorded in the Scientific Registry of Transplant Recipients database were evaluated. Kaplan-Meier analysis was used to compare patient and graft survival among KAP, KA1 and KA2 groups and multivariate Cox regression models were used to assess the adjusted effect of donor and recipient covariates.
Results: Candidates with previous PTA remaining on the kidney transplant wait list have a significantly lower adjusted 3-year patient survival than receiving kidney transplantation (HR = 6.456, 95% CI = 2.646-9.749, P<0.001). The adjusted 10-year kidney graft survival in KA2 patients was the worst (HR=1.197, 95% CI: 1.144-1.252, P<0.001), however, there was no difference between KAP and KA1 (P=0.764). KAP from deceased kidney donors had a significantly inferior 10-year patient survival comparing to those from living kidney donors ( P<0.001)
Conclusion: PTA recipients with ESRD obtained a significant survival benefit after KT compared to those who remain on the waiting list. KAP recipients can achieve similar post kidney transplant survival as a KA1 recipients.
