Background:Pervasive disparities in access to kidney transplantation persist despite decades of recognition and efforts to improve equity in transplant access. Using technology to reduce the gap between dialysis centers and transplant center could reduce barriers.

Methods:SCMJF implemented web-based software platform (Magnus, Magnus Tecnologia) to improve communication and efficiency between dialysis centers and transplant center. We first started using the platform on February 2012. Since than, 19 different dialysis facilities started using the platform online to refer and schedule their patients. We analyzed the time from dialysis initiation to refer to transplant evaluation in the first 4 years using the platform. Analyses of differences between the years were performed with chi-square test.

Results:One thousand one hundred and six patients have been referred to transplant evaluation in our center (193 on 2012; 285 on 2013; 346 on 2014 and 282 on 2015) from 19 different dialysis facilities. Patients referred to evaluation before dialysis initiation rose from 4% in 2012; to 13% in 2015. Patients referred to transplantation after dialysis initiation and before 6 months on dialysis rose from 23% to 26% to 44% to 47% on 2012, 2013, 2014 and 2015 respectively (p<0.05). Patients referred to transplant evaluation after 6 months on dialysis declined form 73% to 68% to 47% to 40 on 2012, 2013, 2014 and 2015respectively (p<0.05).CONCLUSION: Our findings demonstrate that the online platform not only reduced time from dialysis initiation to transplant evaluation but also increased in the numbers of patients referred do kidney transplantation evaluation in 4 years of evaluation.
