Impact of Disease Duration on Islet Yield for Clinical Islet Autotransplants for Chronic Panreatitis

Background: Total pancreatectomy followed by autologous islet transplantation (TP-AIT) is an advanced surgical option for refractory chronic pancreatitis (CP) patients. Islet yield is an important factor to predict post-transplant endocrine function; however, the relationship between disease duration and islet yield has not been clarified.
Methods: A total of 51 CP patients with TP-AIT were included. The patients were divided into four groups according to disease duration (DD) of 10 years and pancreatic calcification (PC) in clinical studies of X-ray, transabdominal/endoscopic ultrasonography or computed tomography. Islet isolation was performed with modified Ricordi method. Total islet yield, islet yield per pancreas weight and islet yield per patient body weight were evaluated.
Results: No significant differences were found in basic characteristics between DD>10yrs and ≤10yrs group for each PC classification (Table).

DD≤10yrs group had significantly higher total islet yield and islet yield per patient body weight for both PC classification, and islet yield per pancreas weight for positive PC (Figure, *p<0.05 and §p<0.1).
Conclusions: Higher islet yield can be expected if CP patients have disease duration less than 10 years when classified by pancreatic calcification.
