Background: Worldwide the population is ageing. In UK there are more people above 60 than under 16 years of age. Hence there are more elderly people coming forward as potential living organ donors. For kidney donation age on its own has never been an absolute contraindication but increasing age and associated co-morbidities can potentially increase risks to donor's health and life. In this study we aimed to analyse outcome of donors above and below 60-years-of-age with associated hypertension.

Methods: We performed a retrospective analysis of outcome of living kidney donors from 1997 to 2016 performed in our unit. We compared the outcome among above and below 60. We used SPSS software for statistical analysis.

Results:

Demographics and outcomes Cohort A < 60 with HTN (n=39) Cohort B > 60 with HTN (n=21) Significance
Gender: Female (%) 51% 60% 0.210
Age in years (median:range) 45.5 (18-59) 63 (60-76) <0.01
BMI (kg/m2) (median:range) 26 (17.7-38) 26 (18-35) 0.480
GFR combined pre-donation (median:range) 94 (70-148) 86 (63.8-119) <0.01
GFR of donor post nephrectomy discharge (median:range) 51 (34.5-74) 48 (33.2-74) <0.01
GFR of donor post nephrectomy 12 months(median:range) 55 (38-148) 47 (33.2-119) <0.01
Haemorrhage 2 1 0.484
Chest infection 4 8 <0.01
Wound infection 2 2 0.241
UTI 4 4 0.523
VTE 1 1 <0.01
Open conversion 1 0 <0.01
Median hospital stay 3 4.5 <0.01

Conclusion: With proper pre-donation workup and consenting, organ donation among elderly donors above 60 with controlled hypertension is a safe option.
